Who Decides When the AI is Wrong?

A few weeks away from posting as life and work have a way of taking over. Switzerland swept me away for a while...

Back now, and this week is about a question that does not get asked often enough.

When an AI system starts producing outputs that look off, not obviously, but subtly and persistently off, who actually makes the call?

Who is the named person, with a specific trigger, and the right to act without booking a meeting first?

Most organisations have not answered that question. This article is about why that matters.
